Medical Sheet
When my son was eight, I found that I was getting so frustrated having to repeat myself at every medical appointment. There was so much information to go over before the nurse or doctor even got to a physical exam. I came up with the idea to compose a one page medical history sheet.
This sheet of paper saved us and the medical professionals an incredible amount of time. On this sheet, I list my son’s surgeries (dates and procedure performed, where it was performed, and by whom), his medications, all doctors including any specialists, their names and contact information, his allergies and our pharmacy information.
As soon as we would walk into an appointment it was handed in with any paperwork they had me fill out in the waiting room. The staff was always so thankful that we showed up so prepared. They would often say how they wished every patient were this organized. This sheet goes everywhere with my son to this very day. I encourage anyone with any medical history to consider keeping this type of sheet and letting loved ones know where to find it in case of an emergency.
